I’d like to enroll in KUA’s Community Solar Program. By checking this box, I agree to sign up for KUA’s Community Solar and agree to the terms and conditions on the attached “Rider – Community Solar Rate” filed with the Florida Public Service Commission. I further acknowledge and agree as follows: I understand that a commitment to the Community Solar program is required.

I also understand that I can cancel my subscription at any time without any penalties, but will not be allowed to return to the program for a minimum of one (1) year and only if solar capacity is available.

I understand that if the application is approved, but the program is fully subscribed, I agree to be placed on the KUA Community Solar program waiting list. When capacity is available, KUA will notify me to continue the subscription process. I understand that I must reply within seven business days after being notified or the next applicant on the waiting list will be offered the opportunity.

I understand that the cost for KUA Community Solar generation is a rate not expected to exceed $.04 per kwh and my solar surcharge will be calculated as follows: Subscription Percentage x Total kWh consumed x Solar Energy Surcharge Rate^
